Update from Your Minnesota Stewardship Collaborative

Antibiotic Stewardship in Ambulatory Care Conference Materials

Summary Report: 2017 MN Clinic Survey of Antibiotic Stewardship Practices

  • A Minnesota Department of Health (MDH) survey was conducted to understand current clinic commitment to antibiotic stewardship, learn about stewardship activities, identify barriers, and define stewardship resource and support needs.
  • Nearly half of surveyed clinics have not made a formal commitment to stewardship or developed a stewardship policy.
  • Minnesota has room for improvement in use of stewardship interventions, like watchful waiting, use of treatment guidelines, and providing patients with antibiotic alternatives.
  • Read the MDH Antibiotic Stewardship Clinic Survey Summary Report.

Summary Report: 2018 MN Community Pharmacist Survey of Antibiotic Stewardship Practices

  • Minnesota Department of Health (MDH), with partners Minnesota Board of Pharmacy and Minnesota Pharmacists Association, surveyed community pharmacists about commitment to antibiotic stewardship, inclusion of stewardship activities in daily workflow, barriers, and resource and support needs.
  • Over 80% of community pharmacists believe that they play a role in antibiotic stewardship and prevention of antibiotic resistance.
  • With only 25% of surveyed pharmacists educating patients about antibiotic stewardship and resistance and only 10% utilizing a pharmacy stewardship policy, there is room for improvement in Minnesota's community pharmacies.
  • Read the MDH Antibiotic Stewardship Community Pharmacy Survey Summary Report.
  • Utilize the MDH Antibiotic Stewardship fact sheet for community pharmacists.

Show your facility’s commitment to antibiotic stewardship!

  • MDH has expanded the Antibiotic Stewardship Commitment Toolkit, which can be used across the continuum of care.
  • Find posters that can be customized with your facility logo, prescriber names and photos, or even patient photos.
  • Commitment posters have been shown to improve prescribing practices, and they provide a great way to discuss responsible antibiotic use with patients and families.

New Guidance Document for Long-Term Care Stewardship

  • With partners from Minnesota's Antibiotic Stewardship Collaborative and beyond, MDH has developed a guidance document called Using Electronic Health Records for Antibiotic Stewardship.
  • The document is intended to support facilities to use electronic health records to drive action and facilitate antibiotic use tracking and reporting.

News to Note

Antibiotic courses for sinusitis often exceed guidelines, per CDC study

  • Sinusitis is the most common condition for outpatient antibiotic therapy in US.
  • For bacterial sinusitis, Infectious Diseases Society of America recommends 5–7 days of antibiotic treatment for uncomplicated cases.
  • CDC found that median duration of therapy prescribed was 10 days, highlighting an opportunity for improvement in outpatient antibiotic stewardship.

Longitudinal assessment published describing long-term care resident MDRO colonization

  • Residents of nursing homes can be more prone to colonization (asymptomatic carriage) with multi-drug resistant organisms (MDRO).
  • In a new study published in CID, authors report that of 651 residents, 56.8% were colonized with MRSA (16.1%), VRE (33.2%), or resistant Gram-neg bacilli (32.0%) at enrollment.
  • Risk factors for colonization included antibiotic use, hospitalization for >14 days, functional disability, and medical device use.
  • Newly acquired colonization occurred at rates of 3.4 days for MRSA, 8.2 days for VRE, and 13.6 days for resistant Gram-negative bacteria.
  • This study highlights the importance of antibiotic stewardship as well as communication about antibiotic use, MDRO infection, and MDRO colonization within and across health care settings.

American Association of Bovine Practitioners (AABP) releases position statement on raised-without-antibiotics programs

  • In an AABP Position Statement, the professional association representing bovine veterinarians recognizes the importance of responsible antibiotic use as a tool in the veterinarian's role of preserving animal health and welfare.
  • The key message of the position statement is, "Programs that seek to market cattle as “raised-without-antibiotics” (RWA) must not compromise cattle health and well-being in order to maintain animals in an RWA status."
  • As RWA production has expanded, there is a growing concern that the practice might compromise animal health and welfare if production and marketing channels for animals that require treatment are not in place.

Answer: e. <1%

According to the CDC "Is it Really a Penicillin Allergy?" fact sheet, approximately 10% of U.S. patients report having an allergic reaction to a penicillin class antibiotic in their past, but fewer than 1% are found to have a true allergy once evaluated. In addition, about 80% of people lose a penicillin allergy sensitivity after 10 years.

Assessment of penicillin allergies can be included in a comprehensive antibiotic stewardship program. In addition to the materials from CDC, check out the slides from Dr. Becca Peglow's presentation at the 2018 Annual MN Antibiotic Stewardship Conference: Focus on Ambulatory Care.
